The economic benefit of using a D-dimer based algorithm in the diagnosis of venous thromboembolism has long been known. In ‘ Health Technology Assessment 2006: Vol. 10: No. 15 ’ more than thirty different algorithms involving various lab and point of care assays and imaging techniques were analysed for cost saving and efficacy. The research found that two of the studies stood out in terms of patient management and cost benefits. One from Wells et al. was shown to offer greatest cost saving at the same time as missing only 3 in 1,000 clinically significant VTE's. The other from Kearon et al. used more extensive imaging for the confirmation but returned similar excellent patient results. Both algorithms involved the use of the Wells scoring system and SimpliRED at the point of care.

It is clear that with a mixture of pre clinical assessment and a highly sensitive and specific assay many of the outpatients who may otherwise have been referred for imaging or other testing were able to be discharged. As some 70-80% of patients who present at ER with query VTE are found to be negative, this sort of screening can provide major financial and care benefits.
